Transfer Center Nurses evaluate, process, track, document, and coordinate patient transfers and patient transports<br />into and out of the Stanford Medical Center. Transfer Center Nurses efficiently engage appropriate departments in the transfer<br />process and are responsible for oversight of the entire transfer episode, from start to finish.<br /><br />Our Clinical Nurse (RN) job classification is represented by the Committee for Recognition of Nursing Achievement (CRONA). Please check AskHR for a complete listing of those cost centers requiring the ACLS certification.<br /><br /><br /><br /><br /><br /><b>Locations</b><br />Stanford Health Care<br /><br /><b>What you will do</b></p><ul><li><p>Contacts Case Manager at insurance or review organization to establish case management.</p></li><li><p>Coordinates emergency admission clinical issues as needed.</p></li><li><p>Coordinates with staff of other hospital departments, including Medical Communications, Case Management, Social Work, patient care units, and outpatient clinics to ensure that the patient's needs are met from admission through discharge.</p></li><li><p>Documents findings and recommendations for accepted transfers or transports.</p></li><li><p>Performs nursing assessment to determine the appropriate level of care at the hospital (i.e., critical care, acute care, observation or outpatient) and the need for specialized equipment, isolation or other patient needs; provides nursing assessment information to patient care unit or clinic accepting the transfer or transport and coordinates the arrival time; assesses, implements, and evaluates appropriate interventions for a patient's current or changing clinical status.</p></li><li><p>Provides an expert level of clinical decision-making to determine patient acuity, need for transfer, appropriate means of transfer, appropriate level of care during transfer or transport, and to sort through all patient information and formulate an accurate picture of the patient and their condition for the hospital's physicians and nurses.</p></li><li><p>Provides nursing care to patients with the more acute or complex conditions or in emergencies in accordance with Transfer Center protocols.</p></li><li><p>Screens patient for anticipated discharge planning needs, such as acceptance of patient back to referring health care institutions, skilled nursing facility (SNF), and/or other community resources to ensure patient and family needs are met.</p></li><li><p>Serves as an expert, resource, and consultant to other nurses, physicians, departments and families of patients.</p></li><li><p>Sorts through all patient information and formulates an accurate and complete picture of the patient and the patient's condition for the hospital's physicians and nurses; confers with Admitting Physician when questions or issues arise regarding medical necessity of transfer; confers with Social Workers or Case Managers regarding patient and family psychosocial needs.</p></li></ul><p><br /><b>Education Qualifications</b></p><ul><li><p>Graduation from an accredited school of nursing</p></li></ul><p><br /><b>Experience Qualifications</b></p><ul><li><p>Five (5) years of progressively responsible and directly related work experience in critical care required.</p></li></ul><p><br /><b>Required Knowledge, Skills and Abilities</b></p><ul><li><p>Knowledge of the Emergency Medical Treatment and Active Labor Act (EMTALA) and related regulations</p></li><li><p>Knowledge of the requirements and regulations of third-party payers, such as Medi-Cal, Medicare, CCS and other government programs as well as private insurance</p></li></ul><p><br /><b>Licenses and Certifications</b></p><ul><li><p>Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure .</p></li><li><p>BLS - Basic Life Support .</p></li></ul><p><br /><br /><b>These principles apply to ALL employees:</b><br /><br /><b>SHC Commitment to Providing an Exceptional Patient & Family Experience</b><br /><br /><br /><i>Stanford Health Care sets a high standard for delivering value and an exceptional experience for our patients and families.
